Sports Physicals
- Physicals are required for participation in all Kenmore East sports programs.
- All parents and athletes are required to sign up through www.finalforms.com in order to participate in all sports.
- The physical should be done by your own doctor.
- Physicals may be scheduled by the health office, per medical director, only during specific times of the year.
- Physicals are valid for one year from the date of the exam.
